Please describe exactly what actions triggered the bug
and the precise symptoms of the bug:

Hi Adrian,

I wonder if you can take a look at this bug. There appears to be two
spelling menu entries. I have taken two screen-shots to show the
problem. It is reproducible with Emacs -Q.
