No, it skips the text elements.

For instance:

<!-- aazzz -->
<zzz>
abzzz
<zzz>aczzz</zzz>
</zzz>

With ispell-comments-and-strings, it just detects aazzz.
With ispell-region, it just detects abzzz and aczzz.

I wish both at the same time.
