Sorry for delay. Reading the stack trace I see there are 3 entries within "fdisk". Are you using GNU fdisk? If yes, could you please provide informations on what have you done?

This indicates that you have a logical dos partition that immediately
follows a previous one, with no gap between for the EBR.
